  • Before submitting this form you must apply, and have signed the promissory note for a new supplemental loan in the exact gross amount of your request. (remember to include the loan fees in your gross amount if you are borrowing from a program that has fees).
  • You cannot use this form to apply for a new supplemental loan. Before submitting this form, you will need to follow the application instructions described in theĀ Education Loans section of our website. You will be ineligible for a computer loan request if you fail to complete the loan application process in addition to submitting this form.
  • After submitting this form, all computer receipts to document your request must either be submitted via the HLS Secure File Upload Form or by coming into the office and using our self-service scanner. Receipts must be in PDF forms ONLY. Photos (JPEGS) taken by a camera or by a mobile device will not be accepted.
